Name Change
Changing one’s name on legal perspective is a very extensive process. A name is a person’s identity, and thus involves various tasks including accurate recordings, identity frauds safeguards and filling up of various kinds of forms.
The types of name changes include:
- Formal name change of adults
- Formal name change of children
- Electing to change your name to a partner’s or spouse’s name
- Assuming a partner’s or spouse’s name
- Resuming your surname
To apply for name change you will need to provide following information:
- Birth certificates and previous name change certificates of all the persons who are applying.
- A statement by a guarantor who personally knows the applicant and can confirm that the applicant has lived in province of residence for at least one year prior to the date of applying.
- Marriage certificate, divorce certificate, annulment certificate, death certificate of spouse or joint declaration of conjugal relationship.
- Copy of notice to your spouse if you are married and are changing your name to another surname.
- Copy of proof of custody and written consent from those who have custody if you are changing your child’s name.
- Copy of proof of notice to every person who is lawfully entitled to have access to the child.
- Written consent of the child, if the child is 12 to 18 years old.
